Welcome to the Baltimore Neurosurgery and Spine Center. In order to best serve you, we request that you do the following:

       Complete the attached Patient Questionnaire and Billing Information Form
          prior to your visit and bring them with you to your appointment;

       If your visit is related to a Work-Related Injury under Workers Comp,
          complete the attached Patient Questionnaire and Workers Comp Form
          prior to your visit and bring them with you to your appointment;

       Review our New Patient Instructions prior to arrival;

       Be sure to obtain actual films and reports in connection with all x-rays, MRIs
          and CT scans and bring them with you to your appointment;

       Bring with you to your appointment all insurance card/s and any additional
          information relevant to your insurance coverage.
